Description: Team Leaders are leaders/mentors for staff and the individuals we provide services to.  You will perform direct client care as well as additional leadship and supervisory duties.

Job Description

A Team Leader functions as a hub of the work team, providing coordination and assuring good communication within the work team.  In this job you will work directly with individuals, teaching life skills and supporting them to led full and meaningful lives.  Some tasks:

  • Follow Kokua policies and procedures and assure that team members also abide by these guidelines.
  • Monitor the work of team members and provide mentoring for team members needing support.
  • Promote accountability and professionalism among team members.  Actively seek assistance from supervisors to help resolve team member conflicts.
  • Participate in the development of agency and state requires care plans.
  • Actively work to support the individuals we serve to meet their goals.
  • Oversee teams staff schedules and review timesheets.
  • Attend Team Leader meetings and actively participate in the process of quality improvement.
  • Carry the Team Leader On-Call phone on a rotating basis to ensure 24-hour response to client needs.
  • Assisting an individual to communicate their needs.
  • Supporting an individual with self-advocacy.
  • Assisting an individual with household tasks and grocery shopping.
  • Assisting with medication management and support.
  • Assisting with behavioral supports.
  • Supporting the work team to meet all of the needs of the individuals living in the home.  The focus of this is independence and an increased ability to advocate for themselves.
